Lamination plate only to be quoted with repair/refit

Warrantees should be honoured as per supplier warrantee period, not less than 24 months

The AMPPRO test determines the activity level of the patient (k-level)

"Special criteria" items requires detailed motivation from MSP and will be adjudicated based on merit and internal

criteria
